At present the mkmk table only covers I Ching combining dots. Expand to cover all the combining accents we support.

The basics (stacking upper accents on top of each other) now basically work; leaving the bug open and shifting to next release because I'd still like to do some fine-tuning and examine non-upper accents, but that will require more design work on the use of the different mark classes, currently only sketched out.
